This booklet serves as documentation for your company’s deliveries.
The driver will complete this form when he delivers a load. One copy you will give to the customer and the other you will keep for your records to go along with the Bill of Lading.
Documentation is always a good thing.
Remember: in this industry if it’s not documented it’s not done.
